突破血脑屏障！新型AAV变体强化递送技术，助力脑疾病治疗！

相信大家对血脑屏障 （BBB） 这个名词并不陌生，通俗来讲，血脑屏障就是大脑的“大门”。大脑作为人体的“机密重地”，掌控着人体的多项重要功能，血脑屏障能够把血液中可能存在的毒素、病原体等有害物质拒之门外，保护脑组织的安全。

由此看来，血脑屏障对保持脑组织内环境的基本稳定，对维持中枢神经系统正常生理状态具有重要的生物学意义。但不幸的是，受到严格把控的血脑屏障也成为了阻碍中枢神经系统 （CNS） 疾病治疗的一道天堑。

2022年10月10日，美国哈佛大学布列根和妇女医院神经外科系 Fengfeng Bei 团队在 Nature Biomedical Engineering 期刊在线发表题为： Variants of the adeno-associatedvirus serotype 9 with enhanced penetration of the blood-brain barrier inrodents and primates  的研究论文。

该研究报告了一种 新型腺相关病毒 （AAV） 变体 —— AAV.CPP.16 ，在临床前动物试验中表现出比以前开发的运载工具更高效的递送效率，能够突破血脑屏障的限制，可用于治疗脑肿瘤 （如胶质母细胞瘤） 和中枢神经系统遗传疾病。

在过去二十年里，人类在基因治疗领域取得了一系列里程碑突破。基因治疗也逐渐成为当前最火热的新兴疗法之一，特别是腺相关病毒 （AAV） 已成为基因治疗的首选递送载体，也被公认为最有前景的递送载体。

在过往的研究中，科学家们已经发现了腺相关病毒 （AAV） 可以在某些情况下穿越血脑屏障，但通常情况下，AAV的这种穿越血脑屏障的递送效率较低，无法满足针对脑部的基因疗法的治疗需求。因此，FengfengBei 团队致力于优化AAV，提高其越过血脑屏障的效率和传递药物的潜力。

在这项最新研究中，为了改进现有的AAV，研究团队将目标锁定在细胞穿膜肽 （CPP） ——一组已知能够穿过生物膜 （如血脑屏障） 的短肽。研究小组收集了大约100种细胞穿膜肽，并将它们插入到各种AAV中，然后对这些重组AVV逐个进行测试，以寻找最有效的。

经过一系列的测试和不断优化，研究团队十分幸运地找到了答案—— AAV.CPP.16 。与常用的AAV9相比，AAV.CPP.16跨越血脑屏障的递送效率显著提高：在不同小鼠品系中提升约6-249倍脑部感染效率，在食蟹猴模型中也有约5倍的提升。

紧接着，研究团队进一步检测了AAV.CPP.16的有效性，他们发现，AAV.CPP.16表现出较高的脑部神经元细胞、星形胶质细胞和脊髓神经元的感染亲和力。在安全性方面，虽然静脉注射AAV.CPP.16仍可感染外周脏器，如肝、肌肉、心脏和背根神经节等，但并未引起显著持续性肝脏毒性反应以及背根神经节退化变性。

那么，是什么原因导致AAV.CPP.16具有较高的跨越血脑屏障的递送效率呢？研究人员发现，AAV.CPP.16并非通过破坏性方式，而是显著提高上皮细胞的转胞吞作用，从而跨越血脑屏障。

更重要的是，Fengfeng Bei 团队还探究了AAV.CPP.16作为基因治疗载体治疗恶性胶质瘤的潜力。使用AAV.CPP.16递送PD-L1的研究表明，相比于AAV9，AAV.CPP.16在恶性胶质瘤微环境中的递送效果提升约17.5倍，显著增加CD8+T浸润和IFNg分泌，减少了Treg细胞浸润，从而有效提升荷瘤小鼠生存率至75%，长期存活小鼠复发瘤模型存活率达到100%。

此外，AAV.CPP.16递送的HSV-TK1疗法结合腹腔注射GCV可以明显杀伤肿瘤细胞，减少肿瘤体积，并提高荷瘤小鼠生存率约至57%。

该研究的通讯作者 FengfengBei 教授表示， 这项研究很令人欣慰，因为距离成功转化AAV递送药物通过人类血脑屏障更近了一步。这项研究也表明AAV可以作为一个有效的系统性药物递送工具，用于对抗胶质瘤或者其他有需求的中枢神经系统类疾病。

Fengfeng Bei 本科毕业于清华大学，硕士毕业于伦敦国王学院，博士毕业于伦敦大学学院，目前是哈佛大学布列根和妇女医院神经外科系助理教授，他的实验室致力于开发AAV基因递送技术，目前正专注于突破血脑屏障。据悉，基于这项研究成果，他还成立了基因治疗初创公司 Brave Bio，该公司目前处于隐身模式，此前曾完成约500万美元的种子轮融资。

总而言之，这项研究通过筛选细胞穿膜肽与不同AVV的组合，获得了一种具有高效跨血脑屏障递送能力的AVV变体——AAV.CPP.16。在小鼠和食蟹猴中，AAV.CPP.16表现出较高的大脑与脊髓的感染亲和力，并在小鼠恶性胶质瘤模型中表现出良好的治疗效果。这一研究为治疗胶质母细胞瘤等脑肿瘤和影响中枢神经系统的遗传疾病打开了新的大门。
